Telehealth Informed Consent
By ordering prescription products through Apotheca by Gabriel, you consent to receive asynchronous telehealth services as described below. Please read this consent carefully before proceeding.
Nature of Telehealth Services
Telehealth involves the delivery of healthcare services using electronic communications. For prescription products on Apotheca, this means:
- You will complete a health intake form providing your medical history, current medications, allergies, and health conditions.
- A licensed physician will review your intake form and requested protocol asynchronously (without a live video or phone call).
- The physician may approve, modify, or deny your requested protocol based on clinical judgment.
- If approved, a prescription will be sent to a licensed compounding pharmacy for fulfillment.
Physician Review
Clinical review is conducted by independently licensed physicians in your state. The reviewing physician exercises independent medical judgment and may:
- Approve your protocol as requested
- Recommend dosage modifications
- Request additional information or lab work before approving
- Decline the protocol if clinically inappropriate
A declined protocol results in a full refund.
Pharmacy & Compounding
Approved prescriptions are compounded by licensed 503A pharmacies that maintain NABP accreditation and comply with USP 795/797/800 standards. Pharmacies are licensed in 48 states. Each formulation includes a Certificate of Analysis (COA) verifying purity and potency.
Risks & Limitations
- Telehealth has limitations compared to in-person visits. The physician will not perform a physical examination.
- Peptide therapies carry risks including but not limited to: injection site reactions, nausea, headache, and allergic reactions.
- Results vary by individual. Peptide formulations are not FDA-approved drugs; they are compounded formulations prescribed off-label by licensed physicians.
- This service does not replace your primary care physician. Inform your regular healthcare provider about any peptide protocols you are using.
Your Responsibilities
- Provide accurate and complete health information on your intake form. Inaccurate information may result in inappropriate treatment.
- Follow the dosing protocol as prescribed by the reviewing physician.
- Report any adverse reactions immediately to both Apotheca and your primary care physician.
- Do not share your prescribed formulations with others.
- Store peptide formulations according to provided instructions (typically refrigerated).
Privacy
Your health information is protected under our Privacy Policy and handled in accordance with applicable healthcare privacy standards. Health intake forms are transmitted securely and accessible only to the reviewing physician and pharmacy.
Right to Withdraw
You may withdraw consent and cancel your order at any time before the pharmacy has compounded your formulation. Contact tyler@askgabriel.com to cancel.
