Head-to-head clinical trials show berberine 500mg 3x/day produces comparable HbA1c reduction (0.5-0.9%) and fasting glucose reduction to metformin 500mg 3x/day. Berberine also reduces LDL cholesterol and triglycerides, an effect metformin doesn't have. However, berberine is a supplement, not a drug, and should not replace prescribed medication without physician guidance.
Take 500mg 2-3 times daily with meals. The key is splitting the dose, berberine has a short half-life (~5 hours) and taking it with each meal maximizes the post-meal blood sugar benefit. Common: GI discomfort, diarrhea, constipation, gas (similar to metformin, usually resolves in 1-2 weeks). Berberine can lower blood sugar significantly, monitor if diabetic.
